About this tool & Important Notice
Universal Phakic IOL Planning & Comparison Tool
The Universal Phakic IOL Planning & Comparison Tool is an educational platform designed to compare the sizing concepts and calculation approaches of different phakic intraocular lens (pIOL) manufacturers within a single application.
The tool enables users to enter available biometric data and review how different manufacturers may approach lens sizing and vault prediction. It provides a structured overview of the characteristics, sizing ranges, calculation parameters, and planning philosophies of various phakic IOL systems, helping surgeons better understand the similarities and differences between them.
Additional educational features include anatomical illustrations, parameter explanations, interactive visualizations, and simulated vault predictions to support learning and training in phakic IOL planning.
Important Notice
This application is intended for educational, training, research, and comparison purposes only.
It is not an official calculator of any manufacturer and must not be used as the sole basis for clinical decision-making or lens selection.
Calculated lens sizes, predicted vault values, and recommendations generated by this application should always be compared with the official manufacturer calculators, current product instructions, and the clinical judgment of the treating surgeon.
The developers make no claim that the calculated results are identical to those produced by proprietary manufacturer algorithms. Final lens selection remains entirely the responsibility of the implanting surgeon.
By using this application, users acknowledge that it serves solely as an educational aid to improve understanding of phakic IOL sizing principles and to facilitate comparison between different manufacturer concepts.

Interactive Toric Planning
Toric Axis Planner
The entire lens body — optic, haptics, toric axis marks — rotates as one physical unit. The required correction axis stays fixed in the eye frame.
ℹHow it works: The outer degree circle and green target axis represent the eye. The central lens symbol rotates physically. Use Auto-Align or the slider to position the lens correctly.

Step 5
Sizing & Vault Prediction
Estimated values only. Always use manufacturer-approved sizing methods.
⚠Important: Vault prediction models here are simplified estimates — not validated. Low vault (<100 µm) and high vault (>1000 µm) carry significant risks. Use manufacturer's own sizing calculator.
Vault — the clearance between the posterior surface of the phakic IOL and the anterior surface of the crystalline lens.
Target range: 250–750 µm.
Low vault (<100 µm) risks cataract; high vault (>1000 µm) may impair aqueous flow and IOP.
Accurate sizing is critical — always use the manufacturer-approved sizing calculator.

Biotech estimates: Park/Igarashi×0.90 (geometry approximation). IPCL: Park formula. Vault target 250–750 µm per general literature.

Why WTW alone cannot perfectly predict vault
Two eyes with identical WTW may develop very different postoperative vaults. Vault depends on the 3D shape of the ciliary sulcus, the crystalline lens position (Lens Rise), and iris configuration — none of which are captured by WTW.
Influence of Lens Rise (CLR)
Higher CLR (e.g. +0.4 mm): the crystalline lens protrudes anteriorly, leaving less space for the phakic IOL → lower vault. Negative CLR: lens sits further back → higher vault. Measurable with AS-OCT or UBM.
Influence of Sulcus-to-Sulcus (STS)
STS is the actual diameter of the ciliary sulcus where phakic IOL haptics sit. Typically 0.3–0.5 mm smaller than ATA. Using STS directly in the Igarashi formula removes the ATA→STS approximation and improves accuracy significantly.
Influence of iris configuration
Convex/plateau iris: effectively reduces the anterior chamber depth available for the phakic IOL — associated with higher IOP risk and lower vault. This correction (−0.10 mm on ACD) is applied automatically when selected.
Architecture: future AI-ready design
The calculation engine, data model, and UI are separated so that new anatomical parameters or AI-based vault prediction models (e.g. neural network models trained on AS-OCT scans) can be added without redesigning the interface.
